Include replicas discovered during refresh in ReplicaCount for request tracing

Currently, the ReplicaCount value for request tracing is set only on startup based on the total connection strings or endpoints passed to the provider. In order to get an accurate count of the number of replicas used, we should use the current replica count during each refresh. This can vary depending on how many replicas were discovered at the time of refresh.
