Document procedure for adding a Stratum 1

There's already documentation for deploying a Stratum 1 server, but we should add the procedure to really add a Stratum 1 to the project, i.e. something like:

• add the URL to the config file(s) (group_vars/all.yml)
• send PR
• generate new client configuration/packages
• add DNS entry?
• update the clients (how do we do/announce this?)

You should use a config repo so that clients pick up new stratum servers immediately.

A tiered config works well: distribute a config package that sets up a config repo, and the config repo provides the info for all the other repos. That way there is rarely a need to install a new config package, usually it will be fine as long as you have one published anytime in the last few months.
