Integrate UTxO-HD

It seems there is already a working node using UTxO-HD. This means it's a good time to start the integration work. Initially we will still use the in-memory version of UTxO-HD. In later iterations we will actually use the full UTxO-HD and take avantage of the memory usage improvements. This issue is only for the first iteration, which is the minimum amount of integration needed.
