[FEATURE] Sql4Cds for Visual Studio Code

Since Azure Data Studio is being retired, it would be great if we could use the extension is VS code instead.

First barrier is installation, which is fixed with a quick update the package.json, but as far as I can tell the "new connection" dialog doesn't exist and therefore the rest of the extension can't be activated.

Any pointers?

This would involve re-implementing a lot of the UI that is currently included in Azure Data Studio - the connection dialog, object explorer, results grid, execution plan view etc.

I don't currently have time to start this project, but if someone would like to pick this up I'd be happy to support where I can.

Rather than targeting VS Code, the new Power Platform Tool Box would be a better fit for this now.
