Update how conversation history is managed in the Smart decision maker block so these issues are fixed.

The prompt and system prompt are added on each loop, this confuses the model.
It's still hard to debug the conversation history.
More effort to do conversation history trimming and context window management.
The SDM on a loop most of the time don't know when to stop
