Notice Template in CentOS Threshold Rule

Default Notice template is as below:

${app} monitoring availability alert, code is ${code}

This parameter ${app} shows centos. I would like to show my hostname/ task name

For example:

${host} monitoring availability alert, code is ${code}

Btw where to see all values in ${code}
