site: allow users to duplicate templates across different organizations

We should add the organizations dropdown to the duplicate template form so that users can duplicate a template and add it to a different organization:

/templates/new?fromTemplate={templateId}

Requirements

• investigate why we use a different form component for this duplicate view - ideally we share the form we use for template creation
• include organizations dropdown and have dropdown default to the current organization
