Retro: PR #3506 — Lock file maintenance (release-v0.8)

This is a Renovate/MintMaker bot-authored PR that updates 4 npm dependencies in package-lock.json (+12/−12 lines): fastq 1.19.1→1.20.1, lodash 4.17.21→4.18.1, picomatch 2.3.1→2.3.2, yaml 1.10.2→1.10.3. The PR was created on Aug 19, agent-approved the same day, human-approved and merged on Aug 26.

Timeline

1. Aug 19 04:07 — PR opened by red-hat-konflux bot, triggering review dispatch via pull_request_target / opened action.
2. Aug 19 04:08–04:17 — Review run #1 (agent from fullsend-ai/agents@v0). Correctness and security sub-agents found no issues; style-conventions sub-agent failed due to model unavailability (info-level, non-blocking). Challenger adjudicated info-level findings. Result: APPROVED.

Workflow quality

The review agent performed correctly — the security sub-agent verified all 4 packages against the canonical npm registry, which is appropriate due diligence for dependency updates. Both runs reached the right conclusion (approve). The main inefficiency is the duplicate review run on the same commit, consuming ~11 minutes of additional agent compute for no new information.

Existing issues — new evidence

All improvement patterns observed here are already tracked by open issues. No new proposals are warranted.

• Skip review dispatch when HEAD SHA was already reviewed and approved fullsend-ai/fullsend#963 ("Skip review dispatch when HEAD SHA was already reviewed and approved"): This PR provides a clean example — the synchronize event on SHA 2f092fd triggered a redundant second review ~20 minutes after the first review already approved the same commit.
• Prioritize same-commit review dedup — PR #944 shows 10x waste on a single SHA fullsend-ai/fullsend#5139 ("Prioritize same-commit review dedup"): Additional evidence of same-SHA waste, albeit minor (2x, not 10x as in that issue's motivating case).
• Skip review and retro stages for bot-authored dependency update PRs fullsend-ai/fullsend#5360 ("Skip review and retro stages for bot-authored dependency update PRs"): This trivial bot lock file PR went through the full multi-sub-agent review pipeline. While the security verification adds value, the overall pipeline cost-to-risk ratio is high for lock file maintenance.
• Add pre-review early exit for bot-authored dependency-update PRs fullsend-ai/agents#257 ("Add pre-review early exit for bot-authored dependency-update PRs"): Defense-in-depth at the agents layer for the same pattern.
• Add bot-PR early exit to pre-retro.sh to skip retro on zero-involvement bot PRs fullsend-ai/agents#276 ("Add bot-PR early exit to pre-retro.sh to skip retro on zero-involvement bot PRs"): This retro run itself is evidence — the retro agent is spending compute analyzing a trivial bot dependency update where the only agent involvement was two routine approvals.
