[BUG] Gate-skipping behaviour with certain models like Kimi K2.5

Description

Certain agents and commands are supposed to have confirmation or user input gates, but they are getting skipped by the model directly generating an answer for certain models like Kimi K2.5.

Steps to Reproduce

1. Install profile: [developer]
2. Run command: /add-context

Expected Behavior

The process guides the user step by step through the setup process.

Actual Behavior

The full process automates end-to-end instead of requiring user input at key points

Environment

• OS: OpenSUSE Tumbleweed
• OpenCode CLI version:1.2.10
• Installation profile: [developer]
• Bash version: 5.4.9

Additional Context

GLM 5 is an example of models that work as expected. It is suspected that template / special token issues might be involved.
