Authoring an ADR

Use 0000-template.adoc as the starting template.
Filename: NNNN-short-slug.adoc (numbering continues from the highest current ADR; pad to 4 digits).
If the ADR promotes a proposal, link back to the proposal file and keep the proposal as the canonical wire-format / detail reference.
