Make yolo mode more obvious and harder to use

I see a bunch of people using yolo mode when they may not realize the risks associated with it. We mention the risks in the settings but it's still too easy to enable. I propose we:

• Show a modal that must be accepted when the setting is changed
• Show a persistent and loud banner in chat views when global auto approve is on
• Change the name to be more obvious and clear that it's not a requirement to use terminal auto approve

The changes landing in August are:

• chat.tools.autoApprove -> chat.tools.global.autoApprove
• A scary dialog shows once a tool tries to auto approve via that setting for the first time. Pressing disable will change the setting to false.

The new Auto approved by setting chat.tools.global.autoApprove disclaimer on every tool use is extremely annoying. There should be a chat.tools.suppressAutoApproveWarning at least.
