Improve the readme

• Show some examples of how you would use this with an express route
• Show example of how you can use a "Custom Callback" to passport.authenticate, so that you can get a reference to the Account object and handle it how you see fit
• Remove info about contributing to the development branch. This is a small repo and we don't need that workflow
