Block.tsx / document bad creation issue

I'm uploading a file, asking for memo based on the upload. The block is correctly activating, but the response is inaccurate. Interestingly enough the AI continues and responded with the correct answer in the chat.

I've tried optimizing the prompt.ts and the app/(chat)/api/chat/route.ts.

Any idea how to get the correct information in the block and the chat?
