Implement `/score` (return current on-chain score & freshness)

Scope

• /score <id> fetches (score, roleCode, updatedAt).
• Reply (ephemeral) with score, role label, updatedAt (humanized), and threshold info.

Acceptance Criteria

• Accurate values for any verified ID.
• Graceful error if ID unknown/not found.
