This ticket resolves existing qa errors found and shall remain open until all May 2026 ballot tickets have been resolved/merged.

Issues resolved thus far:

1. The link 'Practitioner/doc-smith' for "Dr. John Robert Smith" cannot be resolved
2. The link 'Organization/berkshire-family-medicine' for "Berkshire Family Medicine" cannot be resolved
3. The link 'Location/123' for "Berkshire Family Medicine - Pittsfield" cannot be resolved
4. The image source 'ERDiagram2.png' cannot be resolved

1. The packageId hl7.fhir.uv.xver-r5.r4 points to the canonical http://hl7.org/fhir/uv/xver which is inconsistent with the stated canonical of http://hl7.org/fhir/5.0/ImplementationGuide/hl7.fhir.uv.xver-r5.r4
2. Fix duplicative names "Location Physical Type"
3. Add codesystem.caseSensitive to conform to HL7 codesystems
4. Fix missing definition for NDH valueset by swapping it out with THO valueset for AcceptingPatients
