What changes

Clears the remaining findings from the claude.ai directory portal's validation.

• No icon: adds kane-qe/.claude-plugin/icon.png, 1024×1024, drawn from the mascot's pixel art. The portal takes the icon only the first time the plugin is saved or submitted, so this should land before the first submission.
• Unknown field types in plugin.json: removed. claude plugin validate still reads types/index.d.ts by its place in the plugin and checks the mod's state names against it.
• "Takes a permission decision out of the user's hands" (the theme hook): the config.set hook is gone. The mod reads Claude Code's theme every two seconds and redraws only when it moves between light and dark. This commit was pushed to PR kane-qe: chat card badge, shorter cards, folded shell groups stay folded #2 after it merged, so it lands here.
• Image file the code could run (policy hold): the mascot's seven PNG frames are removed. Where the mascot is a picture (the desktop app) it is now an SVG of the same 8×6 pixel poses the terminal draws, one square per pixel, built in code. The plugin ships no image its code refers to, and no embedded image data.
• Listing links: privacyPolicyUrl (https://www.testmuai.com/legal/privacy/) and termsOfServiceUrl (https://www.testmuai.com/legal/terms-of-service/) in plugin.json. supportUrl is still unset.
• User guide: USER_GUIDE.md at the repository root, written for people installing the plugin (install, every surface, testing changes, assurance, evidence, commands, what it runs and sends, limits, troubleshooting). documentationUrl points at it on main, so the link works once this merges.
• The root README's directory section now mentions the icon and the review holds, and says updates are picked up from the tracked branch with no resubmission.

claude plugin validate --strict kane-qe and claude plugin validate . pass; claude plugin test kane-qe: 134 pass; type-check clean. The theme test now changes the theme and advances the clock two seconds.

Live smoke check in a real Claude Code session (fullscreen terminal, demo shop project), with a stand-in kane-cli that fakes a testmd run and passes every other command to the real one: the band (idle, running with spinner, last result, assurance), a passed card and a failed card under a folded group, View steps opening the pane on the run, and the History and Assurance tabs. No drawing was refused.
