fix: honor bin dir alias and Windows wrappers

Summary

• Handle Windows .cmd/.bat/.ps1 wrapper execution in completion hook notifications
• Prefer CODEX_BIN_DIR/CCB_BIN_DIR and install prefix before PATH lookup
• Document CCB_BIN_DIR alias usage in README (EN/ZH)

• Not run locally (CI on push)
