Add hosted Ask AgentOps live response flow

• add a metadata-only live assistant request packet to Ask AgentOps
• render an inline live assistant form in the hosted page when AGENTOPS_ASSISTANT_API_URL or assistant_api_url is configured
• add product audit coverage and docs for the Agent Improvement Loop
