Resolves the merge conflicts from #689 against current main.\n\nIncludes the Alembic migration CLI and docs, ports packaging and dependencies to the current PDM/uv setup, preserves current CI, and replaces deprecated pkg_resources entry-point loading with importlib.metadata.\n\nValidation: uv run pytest tests/test_cli; uv run ruff check sqlmodel/cli tests/test_cli; uv run sqlmodel migrations --help.
