Fix onboarding wizard and LLM error handling - #47

Onboarding now asks about user identity (name, role, how to be addressed), agent personality (name, style, use case, custom instructions), and writes personalized IDENTITY.md, SOUL.md, AGENT.md, and USER.md files. Model validation warns on unknown model names during onboard. Default model updated to gpt-5.3-codex. Known models list includes gpt-5.3-codex-spark. Agent loop now catches LLM provider errors (404, auth, rate limit) and returns a friendly error message instead of crashing with a traceback.
