- README: new "Add the spend firewall" section under the headline showing X-Customer-ID + spend headers; rewritten §3 Per-Customer Spend Limits with tier/project-default/per-customer precedence; added X-Customer-* rows to the Response Headers table; μUSD precision note in the cost-calculation section. - CHANGELOG: fleshed-out [Unreleased] with Added (project defaults, tiers, resolver, auto-provisioning, scripts, tests), Changed (DECIMAL(14,6) widening, %.6f formatting, streaming prompt-size estimate), Fixed (P0-1 streaming bypass, P0-2 downgrade no-op, schema drift).

customer_limits rows are no longer read on the request path.
Migration:
manage_limits.sh still handles API key rate limits, project monthly_cap_usd, and cache settings. Dead Go code (GetCustomerLimit) removed in v0.4.0.
Upgrade
Cache refresh:
Existing monthly_cap_usd values unchanged. X-Customer-Limit-Daily is now six decimals.
