This PR started out as a pass through through the intro lesson READMEs to make the links to other documentation cleaner. Then, I also did the following:

  • Made other changes that I think improve the READMEs, especially in providing a stronger foundation for folks to work the exercises
  • Added lesson titles to the READMEs and made the numbering more consistent.
  • Made various other minor changes (remove compiler warnings, clarified some TODO comments in code, etc.)

There are additional `TODO` comments asking you to insert bounds of nested
for-loops, and fill in `A` and `R` with their respective index values.
