docs: complete the error catalog from the code - #120

The catalog covered execution routes only. Checked every error code the server raises against it on 2026-10-02. - New sections: policy in effect (CAPABILITY_NOT_BOUND, NO_APPROVED_POLICY_VERSION, POLICY_VERSION_UNAVAILABLE) and governance (NON_HUMAN_CALLER_DENIED, SAME_ACTOR_CANNOT_APPROVE_OWN_CHANGE, STEP_UP_AUTHORIZATION_INVALID, PENDING_POLICY_CHANGE_NOT_FOUND, EXTERNAL_CONNECTOR_CHANGE_NOT_FOUND, INVALID_EXTERNAL_CONNECTOR, EXTERNAL_ENDPOINT_ADDRESS_REFUSED, CONFLICT). - AUDIT_UNAVAILABLE row; POLICY_DENIED row names approval, bound signal and policy version refusals. - Error handling and idempotency pages: NONCE_ALREADY_CONSUMED is not reachable over HTTP (a repeated id is a 409 first), as the catalog already said; CONFLICT added to the common codes.
