Restore readable SmartTool rendering

Summary

• Render SmartTools as first-class history cells with the tool name fixed in the header
• Show SmartTool arguments as readable detail rows instead of hiding them in inline JSON
• Preserve generic MCP compact rendering, raw transcript mode, LaTeX/emoji markdown rendering, and other post-0.139 history-cell behavior
