fix(claude): show Fable directly below Weekly

TL;DR

Show the Claude Fable weekly limit directly below Weekly as an enabled, always-visible default for both new and existing installations.

What was happening

• Fable was disabled by default and assigned to the On Demand section, so its weekly limit did not appear alongside the main Claude meters.
• Existing saved layouts retained the old hidden placement even when a formerly optional metric became a new default.

What this changes

• Enables Fable by default and orders it immediately after Weekly in the Claude provider.
• Keeps Fable always visible without changing the existing Claude menu-bar pins.
• Promotes newly seeded default metrics out of previously saved On Demand placement while preserving metrics users already arranged.
• Adds migration regression coverage and documents the updated Claude defaults.

Tests

• swift test --skip-build — 1,323 tests, three skipped, zero failures.
