MCP structured output requires JSON objects. Now we store whether the tool has an output_schema at registration time and use that to decide between CallToolResult::structured() and CallToolResult::success(). Also uses schema_for_output() which returns Err for non-object types, so the output_schema is correctly set to None for these tools.
