Summary

allow azure managed identity auth to use IDENTITY_ENDPOINT and MSI_ENDPOINT metadata hosts when present
validate required headers/secrets for hosted endpoints before making requests
add tests covering managed identity endpoint selection and headers

Testing

go test ./app/auth/plugins/azure_managed_identity -count=1
