[Enhancement]: Improved UI padding, folder structure organization, and README documentation updates

✨ What does this change?

• Added padding-top for theme icons and a slight gap for better layout presentation (theme.css).

• Performed code formatting where needed using simple VS Code extensions for cleaner code.

• Added small documentation comments in index.html for better readability.

• Fixed the duplicate PR template issue.

• Updated folder structure — moved JS and CSS files into their respective folders (/js and /css) for better organization.

• Made multiple README improvements:

  • Fixed the “Play Live Demo” button link — replaced placeholder with the real live demo URL.
  • Added Dark and Light theme demo screenshots for better presentation.
  • Fixed the GitHub Issues link with the actual repository link.
  • Added Your (Author details) (LinkedIn, Twitter, and GitHub) to improve repository ownership and visibility.
